One hard rule
Every code, high or low, must cite a resolving public source URL. A claim without a source cannot be entered. The instrument reads institutions from public records and codes no persons.
The organization
Name.
Founded. Status.
Trajectory. What the organization is and how it has fared, including any decline, suspension, or death, which are coded as data rather than omitted.
Sources. Two or more resolving URLs that establish the case and its trajectory.
The coded dimensions
Code only the dimensions for which you have documentable evidence, and leave the rest blank. Not-coded is a real state and is better than a guess. Score is 0 (absent, or present and failed), 1 (partial or informal), or 2 (present and load-bearing). Confidence is low, medium, or high. The twenty dimension names are listed in the codebook.

The governance cost anchor
Only if the organization publishes its governance and administration cost. Give the annual governance cost per member or per person, with the year, and the source. Leave it blank if no public figure exists.

The adjudication
The maintainer checks that the case is a real institution with a public record, that every code carries a resolving source, and that the anchored scale is applied, then accepts the case into the next quarterly version, returns it to you for revision, or declines with a reason. Every outcome is recorded in the public adjudication log on the tracker page, so you can see where your submission stands. The full set is re-scored every quarter.
